Medicare Enrollment: Step-by-Step Guide

A comprehensive walkthrough of the Medicare enrollment process to ensure you don't miss important deadlines or coverage options.

This guide outlines the complete Medicare enrollment process. Each step includes specific actions to take, documents you'll need, and important deadlines to meet. You can save your progress and return anytime.

1Check Your Eligibility

When to complete: 3-6 months before you turn 65

Before starting the enrollment process, confirm you're eligible for Medicare. Most people qualify for premium-free Part A if they or their spouse paid Medicare taxes while working for at least 10 years.

You're eligible for Medicare if:

  • You're 65 or older
  • You're under 65 with certain disabilities
  • You have End-Stage Renal Disease (ESRD) or ALS

What you'll need:

  • Social Security Number
  • Date of birth
  • Employment history information

2Understand Enrollment Periods

When to complete: 3-6 months before you turn 65

There are specific time frames when you can sign up for Medicare. Missing these periods may result in late enrollment penalties and gaps in coverage.

  • Initial Enrollment Period (IEP): 7-month period that includes:

    • 3 months before the month you turn 65
    • The month you turn 65
    • 3 months after the month you turn 65
  • General Enrollment Period: January 1 - March 31 each year if you missed your IEP

  • Special Enrollment Period: Available in certain situations like if you're covered under a group health plan

Important: To avoid late enrollment penalties, sign up during your Initial Enrollment Period unless you qualify for a Special Enrollment Period.

3Decide What Coverage You Need

When to complete: 2-3 months before enrollment

You'll need to choose between Original Medicare (Parts A & B) or Medicare Advantage (Part C), and decide if you need prescription drug coverage (Part D) or a Medigap policy.

Option 1: Original Medicare

  • Part A (Hospital Insurance)
  • Part B (Medical Insurance)
  • Optional: Part D (Prescription Drug Coverage)
  • Optional: Medigap (Medicare Supplement Insurance)

Option 2: Medicare Advantage (Part C)

  • Combines Part A & B coverage
  • Usually includes Part D
  • May offer additional benefits (vision, dental, etc.)
  • Note: You cannot purchase Medigap with Medicare Advantage

Before deciding, consider:

  • Your current health status and medications
  • Whether your doctors accept Medicare/are in-network
  • Your budget for premiums, deductibles, and out-of-pocket costs
  • If you travel frequently
  • If you have other health coverage (e.g., employer, VA benefits)

4Enroll in Medicare Parts A & B

When to complete: During your Initial Enrollment Period

If you're already receiving Social Security benefits, you'll be automatically enrolled in Parts A & B. Otherwise, you'll need to apply.

How to apply:

  • Online: Apply through the Social Security website (fastest method)
  • By phone: Call Social Security at 1-800-772-1213
  • In person: Visit your local Social Security office (appointment recommended)

What you'll need:

  • Social Security Number
  • Birth certificate or proof of citizenship/legal residency
  • W-2 forms or tax returns for the previous year
  • If you're still working and have employer coverage, a letter from your employer describing your coverage

Important: If you're still working and covered by an employer plan, you may be able to delay enrolling in Part B without penalty. Consult with your benefits administrator.

5Consider Prescription Drug Coverage (Part D)

When to complete: During your Initial Enrollment Period or Annual Enrollment Period (Oct 15-Dec 7)

If you choose Original Medicare and need prescription drug coverage, you'll need to join a Medicare Part D plan. If you go with Medicare Advantage, check if drug coverage is included in your plan.

Important: Even if you don't currently use prescription medications, consider enrolling in a low-cost Part D plan to avoid future penalties.

How to enroll in Part D:

  • Use the Medicare Plan Finder to compare available plans in your area
  • Check if your medications are covered in the plan's formulary
  • Enroll directly through Medicare's website or by calling 1-800-MEDICARE

6Consider Medigap (Medicare Supplement Insurance)

When to complete: Within 6 months of enrolling in Medicare Part B

If you choose Original Medicare, a Medigap policy can help cover some of the out-of-pocket costs like copayments, coinsurance, and deductibles.

Key points about Medigap:

  • Medigap policies are standardized and labeled with letters (A through N)
  • All policies with the same letter offer the same benefits, regardless of insurance company
  • Prices can vary between companies for the same coverage
  • You cannot have both Medigap and Medicare Advantage

How to get Medigap:

  1. Determine which Medigap benefits you need
  2. Use the Medigap Policy Search to find and compare policies in your area
  3. Contact the insurance companies directly to apply

Important: The best time to buy a Medigap policy is during your 6-month Medigap Open Enrollment Period. During this period, you can buy any Medigap policy at the same price a person in good health would pay, even if you have health problems.

7Receive and Review Your Medicare Card

When to complete: After enrollment is processed

Once enrolled, you'll receive your red, white, and blue Medicare card in the mail. This typically takes about 3-4 weeks after applying.

When you receive your card:

  • Verify that all information is correct
  • Make a copy for your records
  • Keep it in a safe place
  • Bring it to all medical appointments

Important: If you don't receive your card within 30 days of applying, call 1-800-MEDICARE (1-800-633-4227).

8Set Up Your Medicare Account Online

When to complete: After receiving your Medicare card

Create a secure Medicare account online to access personalized information and manage your benefits.

How to set up your account:

  1. Go to Medicare.gov
  2. Select "Create Account"
  3. Complete the required information using details from your Medicare card
  4. Create a username and password
  5. Set up security questions

With your Medicare account, you can:

  • View and print a copy of your Medicare card
  • See your Medicare status and eligibility information
  • Manage your personal information
  • View claims information
  • Access your Medicare Summary Notices (MSNs)
